AT&T HTC 8525 PUK Needed?

i was going through the phone settings and exploring my phone. I got at&t htc 8525 Model. i have pressed somthing that has blocked my sim now. its asking me to enter the PUK code. i dont know what to do as i never have even put a code on this phone. somebody tell me how to fix ?

I forgot my iphone lock-code. how can i restore it?
